Vocational Rehabilitation is a process that involves application for services, eligibility determination, vocational counseling and the development of an Individualized Plan for Employment (IPE) to provide services leading to employment.

There are three ways you can access services in the VESID Queens District Office. You can be referred by your treating physician or treatment center, you can apply yourself, or, if you are a high school student, your Transition Linkage Coordinator or your Guidance Counselor can refer you.

Treatment Program or Physician

The Queens District Office has a liaison relationship with many treatment programs in Queens. If you want to be referred for VESID services through you treatment program, you can ask your therapist, doctor, social worker, psychiatrist or other health care professional to send us a referral. They should include as much information about you as they can, especially your diagnosis or disability and how your disability affects your ability to work. If our office has a liaison counselor assigned to your treatment program, you will be assigned directly to a Vocational Rehabilitation Counselor here at VESID who works with your program, once we receive a complete referral packet from your treatment program. If your program does not have a VESID liaison counselor, you will receive an appointment to come to our office for a group orientation.

Apply Yourself

You can apply to VESID Queens by calling us, writing us or faxing us. We will need you name and address and as much of the following information as you can provide:

-Date of Birth -Referral disability

-Social Security Number - Referral source ( who told you about us?)

-Sex -Marital status

-Race

When we receive this information we will send you an appointment for a group orientation.

If you come to our office with medical documentation and decide to apply for services after the group orientation, you may begin the process.

If you do not have access to medical information, you may still come to group orientation, however, we will arrange medical appointments for you to assist us in making an eligibility determination. If you are determined eligible for services, you will be assigned a VESID counselor and will receive an appointment to meet with your counselor.

High School Student

Every high school in Queens has an assigned VESID Vocational Rehabilitation Counselor who receives referral of high school students nearing graduation. The liaison at the high school, usually the Transition Linkage Coordinator, with the student’s permission sends the VESID counselor information about the student. Appointments are arranged for the student to meet the VESID counselor before graduation to begin vocational planning. Group Orientation

Once you contact VESID, you will be asked to attend a group orientation session. You will receive an appointment to attend on a Thursday, either at 9:00 AM or 10:45 AM. The orientation will provide you with a description of the types of services that VESID provides. All the necessary forms needed to apply for VESID services will be given to you at that time. VESID staff will be available to answer any questions you may have or to assist you with the application process.
